Job Description-
We currently have exciting job opportunities within our organisation of Fresh Start Project CIC complex residential support worker roles. Fresh Start Project CIC has 3 sites over the Stockton-on-Tees area and supports males from the local and surrounding Tees Valley areas. The clients we work with have complex and varied needs, whether this be issues with homelessness drug and alcohol dependency; mental health issues, challenging behaviours or past criminal convictions. You will need to be able to work and liaise with other agencies to ensure our client’s needs are met with a multi agency approach.
The post holder must be:
- experienced within a supported housing setting
- able to identify and prioritise support needed/ document record within the clients case records
- have a knowledge of policies procedures required inclusive of health and safety requirements
- Non-judgemental
- provide clear conscise accurate records of key working sessions support sessions evidenced
- signpost and refer into other agencies having a multi agency approach
- be a good team player
- Proactive
- Be able to use initiative and adapt to an ever-changing environment
- Good communication skills
- A great team player- but also able to work alone and use their own initiative
The post holder must be competent in record keeping and have good literature skills to ensure reports and daily notes are accurate and consise .
The Job role will consist of:
· ensure clients adhere to all rules and regulations of Fresh Start Project CIC in line with all policies and procedures within all buildings
· Identifying support needs of the client and signposting them to services to support these individual needs
· Document and Record – ensure that all care plans / records are kept and updated regularly
· Daily building checks – ensuring rooms are free from hazzards and adhering to the organisations policies and procedures
· Promoting independence skills
· Promote health and wellbeing in all day to day working practices/ harm minimisation / drug information
· Advocate for the client for application for benefit claims and phone calls
· To ensure the buildings are fully clean and adhering to infection control procedures
· To organise activities -cooking/ education activities
· Monitor and collect weekly service charges from clients
It would be desirable to have past experience of working with service users with complex needs such as mental health problems and alcohol and drug dependency and also challenging behaviour.
This post is for full/ part time hours over a 7-day period within our Westbourne and Portrack Project.
An enhanced DBS is essential this can be applied for prior to starting the post
Driver would be advantage
