Job Description

MATRIX is a charitable organisation that exists to remove inequalities for children living with an acquired brain injury following and accident or illness, to maximise their recovery, personal development and educational potential; and provide effective support to their families.

JOB PURPOSE:

To support the Case Manager to assess need and plan support interventions that will be

delivered by the Specialist SEN Support Mentors. You will also coordinate access to multi-agency support services as directed by the Case Manager, and provide emotional support to their families and or/carers.

MAIN DUTIES:

•Working with the Case Manager to assess the needs of children, young people and their families, to determine their ongoing development and support needs.

•To help prepare Support Plans based on the needs and aspirations of both the child and

their family.

•Working with parents/carers, to determine the ongoing support needs of the family.

•To coordinate access to all appropriate support services as directed.

•Maintain a good working relationship with families and partners, by maintaining regular

contact.

•To help to monitor and review the Support Plans, preparing timely updates as agreed.

•To ensure accurate records of Case Management information are maintained; ensuring

information is recorded in a professional manner that meets our Data Management

Policy.

•Act as advocate for the child/young person and their family at meetings with multi

agency professionals as directed.

•Provide information on acquired brain injury to children, young people and their family

and/or carers to facilitate greater understanding

•To educate, advise and support parents and other family members of their roles and

responsibilities as long-term advocates for the child.

•To organise and coordinate internal meetings and provide guidance to other case

support staff as directed

•To contribute to external Multi-Disciplinary Team Meetings offering updates and advice

as directed.

•Ensure excellent communication is constantly maintained.

•To undertake any other duties commensurate with the nature and grading of this post or

as directed by the CEO or Case Manager.
