Job Description

South Tyneside Health Collaboration works with GP Practices across three Primary Care Networks (PCNs) in South Tyneside. The PCNs work closely with other community healthcare providers to improve the health and wellbeing of patients.

South Tyneside is looking forward to the next phase in our development of support for Primary Care Networks focusing on the Cancer pathway, and we are looking for enthusiastic, committed, can-do people to join our journey.

The Cancer Care Coordinator will work alongside another Cancer Care Coordinator as part of a small team, and report to the Cancer Care lead who is highly experienced in this field and has a passion for supporting and developing people and will lead and support the team. The role will be key in supporting the networks with the NHS England Early Cancer Diagnosis DES. As this is a new role you will have the chance to input to the service development and how the role will evolve over time. The Care Co-Ordinators will be supporting patients throughout their cancer pathway, helping them to understand the importance of attending their clinical investigations, and implications of their diagnosis and what the next steps are. The roles involve coordinating all key activity including access to services, advice and information, and ensuring health and care planning is timely, efficient and patient-centred.
