Recovery Navigator - County Durham

Working Hours: 37 hours per week, Monday to Friday, 9-5. This role may require occasional evening working to meet service needs.

Contract Type: Permanent

Salary: £25,110 - £32,090

About the Role


As a Navigator, you will play a key role within a multi-agency partnership, supporting individuals involved in the probation system to address dependency-related needs and move towards sustained recovery. Working closely with Probation Teams and partner services, you will provide intensive, person-centred support tailored to each individual’s assessed needs.


This role involves building trusted, professional relationships with individuals on community orders and post-release licences, supporting them to understand and manage substance-related issues, reduce harm, and make positive lifestyle changes. You will work in an outcome-focused environment, delivering meaningful one-to-one interventions and coordinating support that promotes long-term wellbeing and stability.


This role involves frequent travel, with delivery sites across Durham City, Peterlee, Dawdon, Newton Aycliffe and Darlington. 


Key responsibilities

- Manage an allocated caseload, acting as the main point of contact and providing continuity of support

- Carry out initial assessments to identify health, wellbeing, and lifestyle risks, and determine the appropriate level of intervention

- Develop, implement and regularly review personalised recovery and treatment plans with individuals

- Support individuals to progress from initial engagement, through structured treatment, towards self-sustained recovery

- Deliver intensive one-to-one support, building trust and motivation to support behavioural change

- Work to reduce substance-related harm to individuals, families and the wider community

- Maintain effective professional relationships with Responsible Probation Officers and contribute to joint risk management and progress reviews

- Work collaboratively with colleagues, peer mentors and external partners to achieve the best possible health and wellbeing outcomes


About You


We welcome applications from individuals with a range of professional and lived experiences. You may have experience working within:

- Substance misuse or recovery services

- Mental health or wellbeing support

- Criminal justice or probation settings

- A relevant academic or professional background


You will be compassionate, motivated and confident in working with individuals with varying levels of complexity. You will be able to build positive relationships, work autonomously while being part of a wider team, and remain focused on achieving positive, sustainable outcomes.


Please note due to the nature of this role, a full UK driving license and access to a vehicle is essential.
