Outreach Support Worker

This role will be working across floating support projects. You will be working with a caseload of individuals providing support at properties across Darlington. The role involves working within a team but you will be essentially lone working and providing intense outreach support to individuals who have issues such as failing to sustain accommodation, substance misuse, mental ill-health, and who may present with challenging behaviour. The aim is to support individuals to stabilise their situation, assisting them to maintain their tenancy and preparing them for living independently.

Outreach support may include supporting people who do not have accommodation so interagency working is required to ensure a holistic approach to an individual’s support needs.

Staff will also be involved in an out of hour’s on-call rolling rota.

Candidates must have prior experience in supporting individuals with complex needs and lone working.
