&lt;p&gt;We require dedicated, motivated and caring individuals to work with clients throughout Exeter, who have severe and enduring mental health needs and who are moving back into the community from in-patient units.&lt;/p&gt;
&lt;p&gt;Our clients are supported to select their own workers and there is the potential to build up a caseload of regular clients. You will work with our clients who have personal budgets to design services which meet a range of needs including domiciliary care, housing related support and supported living. The work is varied and interesting and is an exciting opportunity to support us in developing our services to be truly client centred.&lt;/p&gt;
&lt;p&gt;We are looking for committed people who are able to work on their own initiative with individuals with varying support needs. The roles demand excellent communication skills as you must be confident liaising with statutory, voluntary and other key agencies. You will work as part of a supportive team with the potential for development and progression within the organisation. This role requires colleagues to deliver personal care and experience of this would be preferred along with an NVQ or equivalent in care.&lt;/p&gt;
&lt;p&gt;Successful candidates will be employed on a zero hour contract and will be required to work on an ‘as' and ‘when' required basis which will include working at short notice. There are no guaranteed hours and no obligation to work hours for which you are unavailable. However, there are opportunities to build up regular hours to suit your personal circumstances. &lt;p&gt;We are looking for a Assistant Project Support Worker to work nights to support the provision of high quality support services in Norwich. This is an interesting role providing night time support to young people living in a supported housing service.&lt;/p&gt;
&lt;p&gt;You will have the ability to work as part of a team with clients to help them to work towards their support plan goals and aspirations by assisting in Key support tasks. With a good understanding of health and safety procedures you will work with a multi disciplinary approach in partnership with clients, statutory and partner agencies and support clients to achieve successful housing and employment outcomes.&lt;/p&gt;
&lt;p&gt;You will have an understanding of the issues that can affect single homeless people and the ability to deliver excellent customer service to our clients. With good communication and IT skills, and be able to work well with colleagues from your own and other agencies.&lt;/p&gt;
&lt;p&gt;Each shift is made up of 6 waking hours (4.75hrs between 19:15 and midnight and 1.25hrs between 07:00 and 08:15 the next morning) plus a 7 hour sleep-in shift. You will work on a rolling rota basis with 3 nights on followed by 6 off. This role involves a high level of lone working so and you will be able to deal with situations in a calm and appropriate manner.&lt;/p&gt;

&lt;p&gt;The service is part of the national improving access to psychological therapies (IAPT) programme and is run by a joint venture between Tees, Esk and Wear Valleys NHS Foundation Trust, County Durham and Darlington NHS Foundation Trust and Mental Health Matters. You will work closely with colleagues to source referrals from within the service as well as from external partners, carry out vocational assessments and compile an action plan with each individual. Working as part of a team delivering both one to one and group based support, you will work closely with employers, training providers and other agencies, supporting a caseload of individuals on their chosen route to gain and retain employment.&lt;/p&gt;
&lt;p&gt;You will be educated to a minimum NVQ Level 3 or equivalent standard, with experience of working with people who have experienced mental health issues and / or other disadvantaged groups having an understanding of the barriers they face in accessing and retaining employment.
